This post will involve working within the Intermediate care Team with strong links to the Community Nursing Team. When planning a programme of care, physical, social and emotional and cultural factors are taken into consideration and suitably qualified staff used to facilitate the level of care agreed. To work as an autonomous practitioner in the planning and implementation of nursing treatment programmes. To advise and instruct carers and patients on health and wellbeing issues, and self-care strategies To carry out specialist and shared complex nursing assessments, to include continuing health care, medication review , prescribing and palliative care.
