To deliver evidence-based holistic professional mental health care to people within the context of an integrated multi-disciplinary, multi-agency integrated community mental health team. The post holder is identified as a skilled and creative practitioner with knowledge of working with and supporting people living within their own communities and will be an integral member of the Community Mental Health Team.
