Home Care Practitioner (Workington)
Home Care Practitioner Part time - 36 hours per week An opportunity has arisen for a Band 3 Home Care Practitioner to work as a member of Workington Integrated Care Community. Working alongside the Community Team with a special interest in Palliative Care and Rehabilitation. We are looking for a motivated and caring individual, preferably with some community experience, but more importantly someone who is enthusiastic about rehabilitation and providing person centred care to people and who enjoys working in a team. The ability to work over 7 days is a requirement of the post. The hours of work will be on a rota covering a shift pattern between the hours of 8 a.m. and 10 p.m. ICCs aim to address the challenges of an aging population and increasing demand on health and care services by providing more out of hospital care and empowering people to take control of their health and wellbeing. This will require new ways of working to focus on preventing ill health and ensure services are sustainable for the future. These new ways of working will follow the same approach across all eight ICCs with flexibility to tailor services to the needs of local people. Workington ICC is a friendly multidisciplinary team, with staff who work together to facilitate early discharge from hospital or prevent unnecessary admission.
