**Job Introduction** -------------------- The SEND Integrated and Assessment Team are responsible for carrying out the statutory functions of the Local Authority with regards to Education, Health and Care Assessment and Review. The Annual Review Officer will work within our Locality Teams and focus on the processing and amending of Education, Health and Care (EHC plans) following a child or young person's annual review. The Annual Review Officer will report directly to the Senior Officer (Team Leader) within the team. The main responsibilities of this role: * To manage and support the timely planning of transfers to the next phase for all children with EHC Plans , to ensure that all change of phase plans are consulted on and amended by 15th February in the year of transfer.
